Advance care planning is when you get to plan for your future health care. If you were to become seriously ill and not able to verbalise your preferences, what health care treatments would you like or not like to have? Thinking about advance care planning gives you the opportunity to discuss and record your preferences for your care including what is acceptable to you. It also enables your family and loved ones to know what’s important to you and ensures your wishes are respected by your care team.
How do you record your choices?
There are three ways you can record your choices for your future health care and personal care here in WA:
- Complete an Enduring Power of Guardianship (EPG)
- Complete and Advance Health Directive
- Complete a Values and Preferences Form: Planning for my future care.
