From 21/12/01 NP P6 of the Privacy Act allows a patient the right to access their personal information
To access your personal information you must complete a request form available at reception.
A doctor will inspect the paper and electronic records. The doctor may request you have an appointment to discuss the records before they are released. This appointment will attract a private fee and Medicare does not rebate this.
You will be required to provide proof of identity on completion of the request form and at the time of collection.
Collating records can be time consuming and consequently there is fee for this service. The fee for records supplied on a disc for an individual person is $10 and for a family $15. If the records are required as a paper copy a fee will be established at the appropriate time. This fee is not covered by Medicare.
