Plan your Future Health Care
Thinking about advance care planning gives you the opportunity to discuss and record your preferences for your care including what is acceptable to you. It also enables your family and loved ones to know what’s important to you and ensures your wishes are respected by your care team.
How do you record your choices?
There are three ways you can record your choices for your future health care and personal care here in WA:
- Complete an Enduring Power of Guardianship (EPG)
- Complete and Advance Health Directive
- Complete a Values and Preferences Form: Planning for my future care.
If you would like to know more, please speak with your doctor at your next visit. We have Advance Care Planning packs available at the practice.
