Continuous quality improvement is at the core of the health care we provide at Quinns Mindarie Super Clinic. Using the ’10 Building Blocks of High Performing Primary Care’ we are constantly working to identify health care gaps and develop strategies to improve the health care of our patients.
We work closely with the WA Primary Health Alliance and meet with other WA General Practices to brainstorm, share ideas and learn from one another. As our whole team-based care expands, we are excited to work alongside other Allied Health Professionals here at QMSC to improve patient care. Our team includes our GPs, nurses, a non-dispensing pharmacist, dietician, medical practice assistant, diabetic educator and social worker (due to start at the end of 2019).
As part of Continuous Quality Improvement, we use de-identified data to monitor the success of our strategies in closing health care gaps and improving health care outcomes.
