Diabetes care is complex and involves routine checks and monitoring. Every 3 months, a HbA1c blood test is recommended. This gives us an indication of your ‘average’ blood glucose levels over that time. When this is not done on time it is called a health care gap.
Here at QMSC, our Medical Practice Assistant (MPA) has been working with 2 GPs to improve this health care gap by identifying their diabetic patients who have not had a HbA1c reading recorded in the last 3 months. Our MPA then works with the patient’s regular treating GP to close this gap and improve patient care.
To best observe the quality improvement data & patterns, we are tracking the % of active diabetic patients who have had a HbA1c recorded in the last 12 months.
Unfortunately our most recent data is trending downwards. This is a flow on effect from the Covid Pandemic with patients opting for telehealth consultations and minimising risk of infection,
